Rather than mine data or documentation for the diagnostic data that drives clinical strategies, we deliver the best results by going directly to the members themselves.The Censeo service suite supports your members and your business goals through a targeted program of physician house calls that is supported by advanced clinical analytics. This has the effect of decreasing reliance on chart reviews or other costly and disruptive interventions to drive HCC scores and quality ratings. We will help you coordinate member and primary care provider outreach while monitoring the network to ensure care delivery and compliant documentation.
